Health of Campers

Health History Form
To be filled out by you and your physician and brought to camp on your child's first day.

Your child should be in perfect health before leaving for camp. If he or she is in any way indisposed, departure should be postponed and the camp notified. If possible, the camper’s session will be rearranged.

The Camps’ Health Centers must be notified in writing of any pertinent medications or diagnoses. Any emotional problems must also be called to our attention. At the time of your child’s check-up, your family physician should provide us with accurate, up-to-date information about your child’s health, and list the purpose, frequency, dosage and instructions for any prescribed medications or overthe- counter medications on a regular or “as necessary” basis.

Campers who are not well upon arrival at camp will be sent home and admission will be postponed. Campers are screened by our medical staff upon arrival before parents leave camp. No camper will be admitted if, upon arrival, he or she is suffering from any skin or respiratory infection, or any evidence of pediculosis, including nits. Pediculosis (head lice) is present in all school systems and may be evident without symptoms. It is imperative that parents should begin checking their child’s head every day beginning three weeks prior to camp in order to leave adequate time for treatment. It is to your advantage to follow this “early procedure” as treatment takes considerable time and re-scheduling campers for late arrival or alternative sessions isn’t always a viable alternative. To facilitate checking your camper, please be sure there are no braids, dreadlocks or the like; they would have to be removed upon arrival. If your camper is found to have lice or nits while at camp, treatment will be based on a case by case basis. Some campers may need to be sent home based on the assessment of our Health Center staff - refunds will also be based on a case by case basis. Check with your doctor to see if your child needs a measles immunization booster.

You must deposit all medications in the camp’s Health Center upon arrival. The camp medical staff will supervise dispensing of all medications. Campers are not permitted to medicate themselves! Bring your child’s prescription medicines in their original containers with the prescription on the container; the Health Centers at both camps stock most over-the-counter medications.

Because of regulations concerning strep throat cultures, we will be sending any such cultures to laboratories in the area. These cultures will only be done if we feel they are essential in determining the exact diagnosis and extent of an illness. The cost for such a lab analysis cannot be included in the session fees and must be borne by the parent or the parent’s insurance carrier.

Accident and illness insurance coverage is included in the camp fees. However, please note this insurance, which has a $50.00 deductible, is a secondary insurance policy. In case of an accident or illness requiring out of camp treatment, you will need to first file a claim with your carrier. This means your insurance benefits have been exhausted prior to the camp insurance becoming effective. Camp’s policy has a maximum coverage of $2500.00. Parents are responsible for paying the deductible and any amount beyond the limit of the camp’s policy. For this reason, please be certain to include your personal insurance plan information on the enclosed medical form. We are asking that you attach a COPY of your insurance card (front and back) to the medical form to expedite treatment and payment of medical costs.
