Log in to your CampInTouch account to download the forms you need.
Please make certain to follow the steps below in order to complete the Camper Health History Forms. Provide us with any and all medical information pertaining to your child, including recent adjustments to medication regimens or planned “Medication Holidays” for the summer. Even if your child is not currently receiving medication, it is important for us to be aware of any behavioral or emotional issues that may have been noticed or dealt with in the past. Remember, all information that you provide is kept strictly confidential.

Please complete PART A. All sections of PART A – CAMPER HEALTH HISTORY are due by May 1, 2015.
Complete the Parent Authorization Form by May 1, 2015.
Print out PART B- CAMPER HEALTH-CARE RECOMMENDATIONS. Complete the top portion and take this to your Physician when your child has his/her physical and have them complete this form.

If any medications are listed on PART B, please see STEP 4. If your child does not take ANY
medication (including OTC and vitamins), you do not need to fill out PART C.
If your child takes any medications, prescription or OTC, you need to have PART C completed by your physician and submitted to the pharmacy as instructed on the form. Any medications listed on PART B should match PART C.
PART B is for camp use only and PART C is for pharmacy use only.
PART A, B, and C(if applicable) are due by May 1, 2015.
If your camper has their physical scheduled for a later date than May 1, just let us know so we can make the appropriate accommodations.
We recommend that you print out and bring a copy of the completed medical form with you on opening day.
